Pam Conlon-Sandhu RN BSN CRRN
Co-Founder - Nursing Leader
Pam is co-founder with an extraordinary depth of experience in rehabilitation nursing, aging care, and clinical operations. Pam is a 5th-generation San Franciscan and 6th-generation Californian, raised in the heart of Silicon Valley. She earned her BSN in Nursing from the University of San Francisco and spent the first 25 years of her career at Mills Peninsula Health Services (Sutter Health), where she led acute rehabilitation as a Certified Rehabilitation Registered Nurse (CRRN).
As Director of Rehabilitation for over a decade, she managed interdisciplinary teams across inpatient care, outpatient clinics, and multiple therapy disciplines. A major highlight of her leadership was driving the development and implementation of Sutter Health’s first Rehabilitation EHR on Epic.
Pam joined Agility Health, a private-duty home health agency, as Director of Client Services. Drawing on her rehabilitation and chronic-care expertise, she guides clients and families through complex medical, recovery, and senior-aging needs as a private care manager.
